VOLUNTEER SIGN-UP FORM


This form is to Sign Up for the BUSC Volunteer Program. It is to be completed and submitted to the Club upon the registration of EACH player, Age Group U8 and above, playing for BUSC.

NOTE: To all Registered Coaches and Assistant Coaches of Division 4 teams and Managers and Treasurers of Division 1 & 3 Team: You do not need to submit the form for reimbursement. You will be paid automatically after the season begins. If you do not receive your rebate by December 20, please contact us.
All Volunteers: If you volunteer and do not receive a rebate by December 20, please contact us. No rebates will be paid after December 31.

You may also choose not to volunteer and to donate your $50.00 Volunteer deposit; but we certainly hope that you choose to provide your assistance to the club. It can be fun!

To qualify for reimbursement, our BUSC Volunteers are required to provide a minimum of four (4) hours of service to the club. More is always appreciated.
